How does the Clinical Health Information Exchange (cHIE) connect my private healthcare provider with my VA healthcare provider?

Signing up for the cHIE is the first step to connecting your VA healthcare provider with other participating medical professionals in Utah.  In order to allow this exchange of information, additional forms need to be completed with the VA to ensure that your health information is shared with your direct consent.
