Knowledge saves lives


With the cHIE, you can have access to clinical information in a secure, electronic manner. Authorized healthcare professionals can access the cHIE through any interfaced Electronic Health Record (EHR) system. To begin, please complete the steps below:


Clinical Health Information Exchange(cHIE) Membership:

Step 1 - Print and Sign the Electronic Commerce Agreement (ECA), Notice of Delegation, UHIN Enrollment Form, and the Clinical Messaging Delivery Request form.

Download and print the UHIN Electronic Commerce Agreement, Notice of Delegation, UHIN Enrollment Form, and the Clinical Messaging Delivery Request. Please read the agreement and form carefully and have the appropriate individual sign it.

Step 2 - Send completed forms to UHIN.

Once your paperwork has been received by UHIN, we will contact you directly to discuss next steps. Should you have any questions, feel free to contact UHIN at 877-693-3071.

Step 3 - Contact your Electronic Health Record (EHR) Vendor.

You will need to contact your EHR vendor to let them know your intention to connect to the UHIN cHIE and forward them our contact information. Also, please provide UHIN staff with your EHR contact information. Our integration specialist will work with your EHR vendor to connect your system to the cHIE.

Laboratories - Labs who want to join the cHIE need to print and sign the Clinical Laboratory SLA.

Download and print the Clinical Laboratory Service Level Agreement. Please read the agreement carefully and have the appropriate individual sign it. Then return the form according to step 2.
